Job Title: Veterans’ Family Case Manager

The full-time Veterans’ Family Case Manager provides support to veterans and their families who have experienced periods of homelessness and/or are experiencing significant risks of homelessness. Utilizing a housing-first approach, the Veteran Family Case Manager focuses efforts on any areas that directly impact obtaining and maintaining permanent housing, and ensures the appropriate documentation and reports are completed in a thorough and up-to-date manner. The Family Case Manager uses the Critical Time Intervention model to prevent recurrent homelessness and other adverse outcomes as a family transitions into the community, while identifying and strengthening their long-term formal and informal support systems.
Education and Experience:
Bachelor’s degree from an accredited college or university in a social-services related field. A minimum of three years of experience in case management or related field experience is required. Prior military service preferred.
