THE UP CENTER
Position: Face Forward Juvenile Diversion Program Manager/Lead Case Manager
DUTIES: Under the direction of the UCB Program Director and within the limitations of agency policy, the incumbent shall execute the following duties:
•Assist in the coordination of the administrative aspects of the Face Forward Juvenile Diversion program including staffing, training, service delivery, and reporting.
•Oversee and participate in ongoing outreach and recruitment of program participants with partner agencies and other organizations.
•Assist in the development and coordination of additional contracts/employer relationships for transitional jobs, on-the-job training, and job placements.
•Provide case management services to the Face Forward participants to include but is not limited to providing proper documentation and records.
•Assist in mentor recruitment and training.
•Facilitate weekly project review meetings to ensure program delivery is in accordance with best practice.
•Supervise utilization of client information system to achieve accurate data collection for program management and reporting.
• Ensure timely and accurate data collection and reporting regarding program participants and outcomes.
•Coordinate with other agencies and organizations to support the needs of participating youth, families and the program as a whole.
•Represent program in the agency’s Continuous Quality Improvement process and report on program progress.
•Assist in the collaboration with the Marketing & Development department, promote mission and cultivate community support from individuals, organizations, foundations and government agencies.
•Other duties as assigned.
Requirements•Preferred: Master’s degree in Social Work, Psychology, Community Development, Programming, or related field with a minimum of three years related experience working with hard-to-serve populations.
•Required: Bachelor’s degree in Social Work, Psychology, Community Development or Programming, or related field with extensive experience in overseeing program development and implementation.
•Preferred: Knowledge and expertise in grant operations and development.
•Preferred: Knowledge in case management services and documentation.
•Preferred: Knowledge and experience in working with at-risk youth and young adults.
•Experience with program development and tasks such as needs assessment, identifying and coordinating community resources, brokering collaborations and partnerships.
•Competence in data collection and reporting processes.
•Willingness to work flexible hours as required.
•Willingness to travel to required Federal grant meetings.
•Must have a valid driver’s license, have a working vehicle and meet background check requirements.
