Job Description Sentara is currently recruiting for a Health Record & Info Analyst. This is a full time evening shift position. Hours are from 11am-8pm.

Job Summary/Purpose:
Prerequisite - Must possess all abilities & skills of Health Record & Information Technician to perform these functions.
Will be responsible to analyze completion of all medical records. Provide support to physicians for completion of their medical record documentation according to the Medical Staff Rules/Regulations, Joint Commission/CMS regulatory standards & patient care/quality needs. Responsible for monitoring all incomplete records & activates suspension process of the medical staff, (Could result in physician's re-appointment to the medical staff & fines). Must be able to facilitate timely investigation and processes to provide coding staff with medical records to code for hospital reimbursement. Will monitor for timely, accurate & delivery of all dictated physician reports for patient care. Responsible for final indexing & quality control function of all medical record images that involves working with interface applications.

· Occasionally subjected to pressure due to irregular hours and stressful situations.

Minimum Qualifications:
High school graduate or equivalent

Experience: Require 2-3 years Health Information experience or medical clerical experience of 2-3 years.
Minimum of two years of office experience.

Licensure: None

Other: Must be able to multi-task in a rapidly changing environment and prioritize tasks. Possess good oral and written communication skills. Good time management and organizational skills with attention to detail. Demonstrated experience working within different windows in a PC environment and good computer skills. Good hand-eye coordination and the ability to meet process document standards as set. Work in duplicate systems during a period of implementation. Knowledge of medical record format (to include all rules and regulations for medical record physician compliance) and content for all patient records. Ability to perform job functions and make decisions without direct supervision. Must possess problem-solving abilities & basic arithmetic skills. Should have developed ability to focus on detailed processes & validate accuracy of others.
Demonstrates the minimum knowledge, skills and abilities to care for the individualized needs of the patient to include physical, psychological, socio-cultural, spiritual and cognitive needs as well as functional abilities including the need for diversified use of such practices.
